Key influencers can use this form to send reports from a tablet or phone.

This is an alternative to the paper forms. We hope that this helps in your work to keep everyone safe on site.

Please only use your phone from an approved place of safety. And do let the HSQE team or your labour manager know if you have any suggestions to make the form better.
